• DS-156: Nonimmigrant Visa Application -- all applicants are required to complete and submit the electronic form with barcode. All applicants for non-immigrant visas at the U.S. Embassy Consular Section may use the Electronic Visa Application Form (EVAF).

Applicants must complete the electronic DS-156 visa application form online; print ONE copy of the form and barcode page. The online DS-156 form does not transmit any information to the U.S. Department of State electronically. The printed barcode contains all of the application information.

When completing the DS-156 EVAF on the Internet at http://evisaforms.state.gov. please pay special attention to these areas:

1. To use the electronic visa application form, your Internet browser must support 128-bit encryption and you should use either an ink-jet or laser printer to print the completed application form. If you are using Windows Internet Explorer, the minimum version that will work with this site is version 5.0, with service pack 2. If you are using Netscape, the minimum version that will work with this site is version 6.2. You must also have Adobe Acrobat Reader installed on your computer in order to download and print the completed application form. The minimum version that will work with this site is version 5. If you do not have Adobe Acrobat, you may download a copy from http://www.adobe.com/products/acrobat/readstep.html
2. Complete the form, press "CONTINUE", print the DS-156 EVAF and two-dimension (2-D) barcode page.

3. Staple the DS-156 EVAF to the 2-D barcode page (and the DS-157, DS-158 if applicable), and bring two (2) copies of these pages to the interview.

• DS-157: Supplemental Nonimmigrant Visa Application -- required for all male applicants between the ages of 16 and 45. This is a non-electronic form and can be filled out manually.
• DS-158: Contact Information and Work History for Nonimmigrant Visa Applicant -- required for all applicants seeking student and exchange visitor (F, M, J) visas and their spouse/children (applicants who have I-20 or DS-2019 form). This is a non-electronic form and can be filled out manually.
